Reye syndrome Acquired / not inherited
Defect / target: Aspirin given during viral illness in children → mitochondrial injury/impaired β-oxidation
Mechanism: Salicylate plus viral illness causes mitochondrial dysfunction with impaired fatty acid oxidation and urea cycle, producing hepatic microvesicular steatosis, hyperammonemia and encephalopathy.
↑ Accumulates: fat in liver (microvesicular), ammonia· ↓ Deficient: ketones, glucose
Presentation: encephalopathy and hepatic dysfunction after aspirin + viral illness; mimic of FAO disorders (Reye-like picture also with valproate)
Labs: hypoketotic hypoglycemia, ↑ NH₃, ↑ LFTs
