Methanol poisoning Acquired / not inherited
Defect / target: Alcohol dehydrogenase converts methanol to formaldehyde/formic acid
Mechanism: Methanol is also an alcohol dehydrogenase substrate; its oxidation yields formic acid that damages the retina and causes acidosis.
↑ Accumulates: Formaldehyde, formic acid
Presentation: Visual disturbance/blindness, anion-gap metabolic acidosis
Labs: Anion-gap acidosis, osmolar gap
Treatment: Ethanol (competitive substrate) or fomepizole
