Hemolytic disease of the newborn (ABO/Rh incompatibility) Acquired / not inherited
Defect / target: Maternal antibodies against fetal RBC antigens (Rh or ABO)
Mechanism: Maternal IgG destroys fetal/neonatal RBCs, overwhelming immature UGT1A1 with unconjugated bilirubin within the first day of life.
↑ Accumulates: Unconjugated bilirubin
Presentation: Jaundice in the FIRST 24 hours of life, anemia, kernicterus risk
Labs: Unconjugated hyperbilirubinemia, positive direct Coombs, anemia
Treatment: Phototherapy, exchange transfusion, urgent workup
Jaundice in first 24 h is always pathologic
