Disulfiram (Antabuse) - acetaldehyde reaction Acquired / not inherited
Defect / target: Aldehyde dehydrogenase (non-competitive, allosteric-site inhibition)
Mechanism: Disulfiram binds aldehyde dehydrogenase at a site other than the active site, lowering Vmax so acetaldehyde from ethanol builds up.
↑ Accumulates: Acetaldehyde
Presentation: facial flushing, headache, nausea, vomiting, hypotension, tachycardia, palpitations after alcohol
Treatment: used therapeutically as aversive therapy for alcohol use disorder
Non-competitive: lowers Vmax, Km unchanged
