Drug displacement of bilirubin from albumin (salicylates, sulfonamides) Acquired / not inherited
Defect / target: Drug target: albumin binding sites for unconjugated bilirubin
Mechanism: Salicylates and sulfonamides compete with unconjugated bilirubin for albumin, raising free bilirubin that can cross the immature neonatal blood-brain barrier.
↑ Accumulates: Free (unbound) unconjugated bilirubin· ↓ Deficient: Albumin-bound bilirubin
Presentation: Increased kernicterus risk in neonates given or exposed to these drugs
Treatment: Avoid salicylates/sulfonamides in neonates
