ALDH2 deficiency ('Asian flush', rs671) Autosomal dominant
Defect / target: Aldehyde dehydrogenase 2 (mitochondrial) polymorphism rs671
Mechanism: Impaired acetaldehyde clearance after drinking causes acetaldehyde accumulation (protein/DNA adducts, mitochondrial damage), mimicking the disulfiram reaction.
↑ Accumulates: acetaldehyde
Presentation: facial flushing, nausea after small amounts of alcohol; ↓ risk of alcohol dependence but ↑ risk of esophageal cancer with drinking
