Acetaminophen hepatotoxicity in chronic alcohol use (CYP2E1/NAPQI) Acquired / not inherited
Defect / target: Induced CYP2E1 (MEOS) converts acetaminophen to toxic NAPQI
Mechanism: Chronic alcohol induces CYP2E1, so standard acetaminophen doses generate more NAPQI, which depletes glutathione and causes hepatocyte necrosis; induction persists for weeks after abstinence.
